Who We Support

We work with older adults and their families across Perth who are:

  • Wanting to stay living safely and independently at home for longer
  • Experiencing changes in mobility, balance, or confidence moving around their home
  • At risk of falls or have had recent falls or near-misses
  • Recovering after a hospital stay or illness and needing support to rebuild independence
  • Living with dementia or cognitive changes that affect daily function
  • Managing chronic pain, arthritis, or neurological conditions
  • Transitioning from NDIS to aged care support as they approach 65

Funding Pathways

From 1 November 2025, the Support at Home program replaces the Home Care Packages and Short-Term Restorative Care programs.

If you have an aged care assessment and are approved for Support at Home, occupational therapy can be included as part of your care plan.

Some clients may also access separate funding for Assistive Technology and Home Modifications (AT-HM).

FAQs

What is the Support at Home Program?

The Support at Home program is a government-funded initiative that replaced Home Care Packages on 1 November 2025. It provides tailored support services to help older Australians live independently in their own homes, including occupational therapy, personal care, nursing, equipment, and home modifications. The program uses a more flexible funding model that can adjust as your needs change.

Do I need a referral to access occupational therapy under the Support at Home program?

No formal referral is required. Once you have Support at Home funding approved, you can contact us directly to arrange an OT assessment. Your Support at Home provider or care manager can also refer you to us, but you’re free to choose your own occupational therapist.

Are home modifications included in the Support at Home Program?

Home modifications can be funded through Support at Home, and some modifications may also be eligible through the separate Assistive Technology and Home Modifications (AT-HM) funding stream. During your OT assessment, we’ll identify what modifications are needed and advise on the appropriate funding pathway. Common modifications include grab rails, ramps, improved lighting, and bathroom adaptations.

What if I don't have funding through the Support at Home Program?

If you don’t yet have Support at Home funding, the first step is contacting My Aged Care on 1800 200 422 to start the assessment process. If you prefer not to wait or want to arrange services independently, we also offer private aged care OT services that you can access on a fee-for-service basis.
