How the Process Works

Referral or enquiry
You or your provider can contact us directly.

Assessment
We visit you at home to understand your goals and environment.

Recommendations
You’ll receive a practical report outlining what can be funded under Support at Home.

Implementation
We can coordinate with your provider or suppliers where needed.

Funding Pathways

From 1 November 2025, the Support at Home program replaces the Home Care Packages and Short-Term Restorative Care programs.

If you have an aged care assessment and are approved for Support at Home, occupational therapy can be included as part of your care plan.

Some clients may also access separate funding for Assistive Technology and Home Modifications (AT-HM).
