• Prescription of mid-range assistive technology (e.g. ramps, rails, pressure cushions, adjustable beds)
• Recommendations for moderate home modifications
• Implementation advice and follow-up call to review outcomes

(High Complexity eg: Home Modifications)

Best for:

Older adults with degenerative or rapidly changing conditions requiring more clinical expertise such as seating and complex home modifications.

Includes:

• All services from the Specialist AT & Cognitive Assessment package
• Post-implementation reviews and reassessments
• Recommendations for higher-level home modifications (e.g. structural or bathroom rebuilds)
• Collaboration with builders or other professionals where required

(High Complexity)

Best for:

Older adults with progressive neurological conditions (e.g. Parkinson’s, MS, MND) or advanced dementia.

Includes:

• Comprehensive functional and cognitive assessment
• Prescription of complex assistive technology (e.g. hoists, hospital beds, pressure mattresses, wheelchairs)
• Carer and staff education in safe use and setup
• Report suitable for GP, allied health, or provider coordination

Outcome: A clear, clinically justified plan to support complex care needs safely at home.
