Assistive Technology and Home Modifications Scheme

The Assistive Technology and Home Modifications (AT-HM) scheme is part of the new Support at Home program, which commenced in November 2025.

It provides separate, short-term funding to help older adults access essential equipment or home modifications that improve safety and independence.

What the AT-HM scheme covers

Under this scheme, older adults can receive funding for:

  • Assistive technology such as walking frames, wheelchairs, shower chairs, and mobility aids
  • Home modifications such as grab rails, ramps, or bathroom access improvements

All home modifications must be prescribed by a qualified occupational therapist, and only where the change is necessary for safety or function, not cosmetic improvement.

These services are guided by the Department of Health, Disability and Ageing’s AT-HM Scheme Guidelines (October 2025), developed under the Aged Care Act 2024.

  • Liaison and coordination – working with suppliers, builders, and care teams to support smooth implementation.

  • Follow-up support – ensuring recommended solutions are practical, effective, and meet the client’s goals.

What you might access AT-HM funding for

You might want to consider accessing AT-HM funding for:

  • Grab rails or a small ramp at your front entry
  • A shower chair, over-toilet frame, or mobility aid
  • Doorway widening or bathroom layout changes to improve safety

How to access AT-HM funding

If you already receive Support at Home services, speak to your care manager or My Aged Care about an AT-HM referral.
